ITB and left knee pain during marathon training

Hi,

I'm new to Endorphins but so happy to be here! I've been running for a few years now and in 2021 while training for the NYC marathon, I started feeling left knee pain when hitting 15+ miles. I took time off, went to PT, and strength trained (ST) to work back to running several half marathons successfully without pain.

This year, I'm training for NYC again and am now dealing with similar IT band issues that're manifesting as left knee pain. Some days it's minimal pain and I can run 4 miles, some days I can run 8 miles, and the other week I ran 16 miles with 4/10 knee pain. I've really tampered down my weekly mileage in lieu of ST.

My PT has said it's driven by hip and back flexibility and probably strengthening my left side generally speaking. He claims it's a tendinitis but may also just be classic runners knee. The pain usually hits my tibia area and feels like a compression like sensation.

I'm doing ST 3-4x a week now targeting my glutes, hamstrings, low back, hips, quads, calves, etc. What’s frustrating is that I’m not quite seeing results from ST, and it’s been 3-4 weeks since I’ve lowered my weekly mileage and upped my ST.

Anything that's worked for you in the past when dealing with these issues? I feel like my cardio is ok to run but my knee is the issue now. Thanks!
