How to decide on deferring entry

It makes me so sad to even consider this! For background, I have been dealing with an on and off knee injury since May. Since then I have yet to feel consistently better for longer than 2 weeks in a row. I started physical therapy in June, but my insurance has denied any further visits because my condition is considered “stable and not medically necessary” so it’s harder without that support! I had xrays done and they looked good, but have yet to get an mri or any other scans. I also have osgood-schlatters that has never really caused problems until now.
I am registered for the Detroit Free Press Marathon on October 15th (9 weeks out) and have wanted to do this for so long. I raced my first half in April and wanted go for the full next. I followed the 5 week base plan and then have been running on and off depending on pain, but never as consistent as I used to be. I think I am more afraid of running and getting injured than anything and it’s causing me to be hesitant and nervous about it all.
What should I consider before deferring my registration? How to stop being afraid of running? TIA for any advice 🫶🏻
